1. INTRODUCTION
Trust Medical + Medical Connect LLC ("we," "us," "our") provides a platform connecting users with licensed healthcare providers for the purpose of obtaining legitimate medical documentation. Our service is intended for mild illnesses and routine absences only.
2. SCOPE OF SERVICES
a) Medical Documentation Services:
- Verification of mild illness or condition
- Legitimate absence documentation for work or school
- Direct communication with licensed healthcare providers
- Secure document delivery
b) Service Limitations:
- NOT for emergency medical conditions
- NOT for severe illnesses or chronic conditions
- NOT for backdated documentation
- NOT for court appearances, probation, or legal matters
- NOT for disability or FMLA documentation
3. ELIGIBILITY AND USER REQUIREMENTS
You must:
- Be at least 18 years old
- Provide accurate and truthful information
- Affirm that all information you provide to Trust Medical is complete, accurate, and true to the best of your knowledge. You acknowledge that providing false, misleading, or incomplete information may result in the termination of services without refund and may subject you to legal liability.
- Have the legal capacity to enter into these terms.
- Not use the service for fraudulent or unlawful purposes.
4. MEDICAL SAFETY REQUIREMENTS
DO NOT use this service if you experience:
- Fever over 103°F
- Difficulty breathing
- Chest pain
- Confusion or disorientation
- Severe headache
- Symptoms lasting over 10 days
- Any other severe symptoms
5. PRIVACY AND CONFIDENTIALITY
We protect your privacy according to HIPAA requirements:
- We only share approved absence dates and return-to-work/school dates
- We do not disclose specific medical conditions or diagnoses
- All communication is encrypted and secure
- Records are maintained according to medical privacy laws
6. PAYMENT AND REFUND POLICY
a) Payment Terms:
- All fees must be paid upfront
- We accept major credit cards and approved payment methods
- Prices are subject to change without notice
b) Refund Eligibility:
- Full refund if provider determines you don't qualify for documentation
- Full refund if documentation is rejected by employer/school
- $30 administration fee applies to cancellations before provider consultation
- No refunds after documentation is issued
7. PROHIBITED ACTIVITIES
Users may not:
- Submit false or misleading information
- Attempt to obtain backdated documentation
- Modify or alter provided documentation
- Share or resell medical documentation
- Use the service for legal proceedings
- Use the service for disability claims
- Harass or abuse healthcare providers
8. LIMITATION OF LIABILITY
Trust Medical:
- Does not guarantee acceptance of documentation by third parties
- Is not liable for any consequences of documentation use
- Makes no warranties regarding service outcomes
- Limits total liability to fees paid for services
9. DISPUTE RESOLUTION
All disputes shall be resolved through:
- Initial good faith negotiation
- Binding arbitration if negotiation fails
- Venue in Delaware courts for any legal proceedings
- Class action waiver applies
10. ACKNOWLEDGMENT
By using Trust Medical, you acknowledge that:
- This is not a substitute for emergency medical care
- Documentation is based on provided information
- Providers make independent medical determinations
- False information may result in service termination
- Documentation acceptance is not guaranteed
11. MODIFICATIONS
We reserve the right to modify these terms at any time. Continued use of the service constitutes acceptance of modified terms.
